How It Works:
Eligibility:

Must have proof of brain injury from a medical provider

Must not receive assistance from any Medicaid Waiver (e.g. Acquired Brain Injury Waiver, Acquired Brain Injury Long Term Care Waiver, Home and Community Based Waivers, and the Michelle P. Waiver)

Must require 24/7 supervision/assistance

Must reside in Boone, Kenton, or Campbell county
Assistance Options:

In-home respite care for a yearly total not to exceed $1,700 (equivalent of 1x four-hour service/month)

Day program care for a yearly total not to exceed $1,000

Apply for the program

Level of Care Determination

Homebound individuals automatically qualify for in-home respite services from a partner private duty agency.

Individuals who are not homebound must provide proof of a phone call or visit to a local day program.

Individuals may also be directed to the NKADD to determine if they qualify for Medicaid Waiver services.

Assistance

Once approved, the Program Coordinator will assist with establishing services.

All services will be billed directly to BIANK.

Reapply Annually
-
If approved for respite care assistance, the set amount of funds will be available to the applicant for a one-year period. Once those funds are exhausted, the applicant will have to wait until the next year. Brain injury survivors are welcome to reapply annually for funding.
